MAY 2018                                                        Brashear, Missouri

FREE SPORTS PHYSICALS

Free sports physicals will be provided for area athletes on May 30 from 9:00 a.m. to 6:00 p.m. and on May 31 from 7:00 a.m. to 12:00 p.m. at the Kirksville High School. Athletes must take the MSHSAA physical form signed by a parent or guardian to the clinic. If the physical is not signed by a parent or guardian, they will not be able to perform the physical. If your child plans to participate in middle school softball or football (Knox County), softball, baseball, cheerleading, basketball, or track during the 2018-2019 school year, your child MUST have an updated MSHSAA pre-participation physical evaluation on file with the district before he/she can begin practice. If he/she does not have a physical, he/she will not be allowed to participate. Student signatures are required on pages 1, 3, and 4. Parent signatures are required on pages 1 and 4. Be sure to mark your calendar so that you can take advantage of this wonderful opportunity. Students may pick up the MSHSAA physical forms from the high school office or online at http://www.mshsaa.org/resources/pdf/PreParticipationPhysicalEvaluation_1617.pdf. Forms will also be available onsite.
